How To Be a Sponsor

Step 1: Select a sponsorship level from the options listed below.

Step 2: Notify Sharon Ellis, by email, of your desire to be a sponsor.  She will coordinate activities to ensure the signage is created and you get the full visibility and recognition you deserve for your generosity. Email: Sharon@jaxtruebluenfb.org.

Step 3: Submit payment.  You can pay by credit card (choose option below) or pay by check. Please make checks payable to True Blue: NFB and mail to: PO Box 1413, Ponte Vedra Beach 32004-1413

Step 1: Complete and submit a registration form. You can register as an individual or a foursome. Please fill out the appropriate registration form completely and return it to Sharon Ellis.
